REPLACEMENT OF LOST OR STOLEN PASSPORT FOR APPLICANTS AGE 16 AND OVER For expedited service (less than 30-day service), you must have the following documents executed by a Passport Acceptance Agent (either an authorized Main Branch U.S. Post Office or Local Clerk of the Court). Applicants must make a personal appearance before a clerk at the Post Office or Courthouse authorized to execute passport applications. For the list of Passport Acceptance Facilities authorized to execute passport applications in your area, click here . This is to verify identity and to determine passport eligibility. All the documents should be officially sealed in an envelope by an authorized Passport Acceptance Agent. Please present the following to the Passport Acceptance Agent:
A. Proof of U.S. Citizenship -Provide one of the following: 1. Previous U.S. Passport 2. Certified Birth Certificate issued by state, county or city with raised seal(if born in US) 3. Certificate of Citizenship or FS-240, DS-1350 (if born outside the US) 4. Certificate of Naturalization issued by the Immigration and Naturalization Service B. Passport Application Form (DS-11 ) completed and signed in front of the Passport Acceptance Agent. Please type or print legibly in black ink when completing all sections of this application. Do not sign the application until asked to do so or you will have to fill out another. It is recommended by the U.S. Passport Agency that you fill out the DS-11 application form on-line, click here . After you complete the form on-line, please print C. Statement Regarding Lost or Stolen Passport (DS-64 ) complete and sign D. Two recent original passport size photographs (2” x 2”) on a white or clear background (click here for the regulations for acceptable passport-type photograph) E. Signed letter of authorization. Must have an original signature. F. Proof of Departure (one of the following) 1. Copy of the flight itinerary 2. Copy of the airline ticket G. Proof of Identity. Proof of identity with any one of these, if you are recognizable: 1. Previous U.S. passport 2. Naturalization Certificate 3. Current, valid a. Driver's license b. Government ID: city, state or federal c. Military ID: military and dependents
NOTE: Your Social Security Card does NOT prove your identity.
If none of these are available , you will need: 1) Some signature documents, not acceptable alone as ID
And
2) A person who can vouch for you. He/she must
1. Have known you for at least 2 years, 2. Be a U.S. citizen or permanent resident, 3. Have valid ID, and 4. Fill out a Form DS-71 in the presence of a passport agent.
NOTE: If the identification card presented to the authorized passport agent was issued less than 1 year from the date of the passport execution, please submit copies of additional forms of ID. G. The government rules require the sealed envelope to be submitted to the Passport Agency within 5 business days from the day it was officially stamped and sealed by the clerk of the Post Office or Courthouse. Requirements and fees are subject to change without prior notice.
